America and that you are not a member of any organization or group which has any part of its
program that attempts to overthrow the government of the United State or any of its political
subdivisions by force or violence. You certify that you have never been convicted of a felony,
assault while participating in a felony, going armed with intent, terrorism, or willful injury to another.
You are not addicted to the use of alcohol or any controlled substance, nor do you have a
history of mental illness or repeated acts of violence. If admitted to membership you will
faithfully endeavor to fulfill the obligations of good sportsmanship and good citizenship,
and, will abide by the provisions of the By-Laws of the Association.
